MISSION STATEMENT
It is our hope that during your time at the University of Hawai‘i at Hilo, you maintain a successful and injury-free career. However, in competitive athletics, injuries do occur and that is where we step in.
We serve as the initial contact between student-athletes and team physicians in regards to injury management. It is imperative to report any and all injuries to a staff member, which can then provide appropriate care. The staff is also responsible for assisting athletes with scheduling physician appointments.
We hope that all of our student-athletes have a successful and enjoyable experience while attending Hawai‘i Hilo, and it is our goal to provide the best possible care for any medical issues that may arise during this time.
ANNUAL REQUIREMENTS
Both new and returning student-athletes must complete the following forms and readings.

ADDITIONAL INSURANCE INFORMATION
Student-athletes are required to take a physical prior to any athletic-related activity and provide proof of medical insurance coverage. Mandatory and applicable medical paperwork must be completed in full and mailed to the Department of Intercollegiate Athletics prior to physicals along with a copy of both sides of your medical insurance card.
Mainland student-athletes who have Kaiser Permanente Insurance must obtain a Kaiser Hawaii ID number. Please contact the Kaiser-Hawaii Region Customer Service Center at 1-800-966-5955 (Press # 1) and have your Kaiser card available. A Hawaii ID number will be provided to you at no charge.
